Seizures Refractory to Standard Anticonvulsants:
Loading dose: 3 mL/kg/hr (150 mg/kg/hr) of a 5% solution in NS or D5W x 3 hours
Maintenance dose: 0.4 mL/kg/hr (20 mg/kg/hr) of a 5% solution in NS or D5W

PR: 0.3 mL/kg/dose (300 mg/kg) Q4 to 6H PRN. Dissolve dose in equal volume of mineral oil prior to administration. Irregularly absorbed by rectal route.

50 mg/mL (0.05 g/mL) (5% solution)

For IV administration: Add 2.5 mL (2.5 grams) to 50 mL of NS or D5W to make a 5% solution. IV tubing (and syringe if plastic) should be replaced every 4 hours.
Syringe and tubing must be protected from light by wrapping.
Administration:
Administer via syringe pump (glass syringe preferred).



COMMENTS:
Routine use discouraged except for treatment of status epilepticus resistant to initial therapy with phenobarbital, phenytoin, diazepam or lorazepam.

Adverse Effects:
pulmonary edema and hemorrhage, hypotension, local irritation, displacement of bilirubin from albumin.

Contraindications: pulmonary or hepatic disease.
