Standard Prescription

magnesium sulfate __mg IM/IV Q__H x __ doses (__mg/kg/dose)

magnesium sulfate __mg IV over 20 minutes x 1 dose (__mg/kg/dose)



Dosages

Hypomagnesemia or Hypocalcemia:
25-50 mg magnesium sulfate/kg/dose
(2.5-5 mg elemental magnesium/kg/dose)  IM/IV every 6 hours x 2-3 doses PRN.
Maximum: 2 g magnesium sulfate/dose  (200 mg elemental magnesium/dose)
Daily Maintenance:
30-60 mg magnesium sulfate/kg/24 hr (3-6 mg elemental magnesium/kg/24 hr IV)  Oral dosing will be found under "Magnesium supplements, oral"

Severe acute asthma:
25-50 mg magnesium sulfate/kg (2.5-5 mg elemental magnesium/kg/dose)  IV over 20 minutes x 1 dose
Maximum: 2 g magnesium sulfate/dose (200 mg elemental magnesium/dose)

Injection: 50% (500 mg/mL) - 10 mL

Standard concentrations for Critical Care:
PICU: 10 mg/ml, 100 mg/mL, 200 mg/mL

Comments

Each mL contains 500 mg magnesium sulfate = 50 mg elemental Mg =  2 mmol (4 mEq)

Rapid IV administration may cause arrhythmias, hypotension, respiratory and CNS depression.  Maximum infusion rate:  125 mg MgSO4/kg/hr (0.5 mmol/kg/hr)

Patients who waste magnesium in urine or stool may require much larger doses, but the adverse effect of diarrhea from oral magnesium may be dose limiting

When using IV doses, monitor serum magnesium level daily

Use with caution in renal failure
