Mechanism of action:

Long-acting amide local anaesthetic agent.

Injection: 0.2% (2 mg/mL), 0.3% (3 mg/mL), 0.5% (5 mg/mL), 1% (10 mg/mL)

Peripheral nerve/Caudal block (Preservative Free):
Peripheral Nerve Single dose:
0.25 - 3 mg/kg/dose. Max 200 mg/dose
Caudal Block Single dose:
0.5 - 2 mg/kg/dose. Max 100 mg/dose
Infusion:
0.1 - 0.4 mg/kg/hr
Max 0.2 mg/kg/hr if ≤ 6 months age

Continuous Epidural Infusion (Preservative Free):
<= 6 months age:  0.1 - 0.2 mg/kg/hr
> 6 months age: 0.1 - 0.4 mg/kg/hr
Solutions containing preservatives must not be used for epidural infusions or caudal/peripheral blocks.

Duration of action 3-10 hours.

Risk of resuscitation-resistant cardiotoxicity with toxic doses or inadvertent intravascular injection.

Treatment of cardiotoxicity includes standard resuscitation measures and lipid emulsion bolus and infusion doses (see intralipid monograph).
