injection: 500 mg imipenem and 500 mg cilastatin per vial All doses are based on imipenem content

Children:  
15 - 25 mg/kg/dose IV Q6H
Maximum:   1000 mg/dose

Adults:
500 - 1000 mg IV q6h
Maximum:  1000 mg/dose

Not to be used for meningitis.

Cilastin blocks the metabolism of imipenem in the kidney.

Dose adjustment required in renal impairment.

Doses greater than 500 mg have a higher incidence of nausea/vomiting; slower infusion rates may decrease incidence of nausea/vomiting.

Seizures may occur, especially in those with underlying CNS disease, with overdose in patients with renal failure and with other predisposing factors.

Meropenem causes less nausea and vomiting and has a decreased risk of seizures compared to imipenem.
