HYDROCHLOROTHIAZIDE (Paeds) (HydroDiuril, HCTZ) Mechanism of action: Thiazide diuretic Ref: 44, 192 Last update: 2019-03-12	tablet: 25 mg, 50 mg suspension (BCCH): 5 mg/mL	Infant < 6 mo.: 1-2 mg/kg/dose PO BID to max. 37.5 mg/day Infant > 6 mo. & child: 1 mg/kg/dose PO BID to max 100 mg/day (child) Adult: 25-100 mg/24 hr PO daily or divided BID to max. 200 mg/day	Monitor serum electrolytes; May cause hyponatremia, hypokalemia, hypomagnesemia. Loss of efficacy at GFR<40 mL/min; avoid use if GFR <10 mL/min Onset within 2 hrs; duration 6-12 hr Standard Prescription: hydrochlorothiazide__mg PO__(frequency) (__mg/kg/24hr)
